All of my cable is RG6. Most of it is fairly new. One section was at least 6 years old, however, so I replaced it.

That did help. Before my output was about 60% pixelated. Now ...

I just rebuilt with the OpenGL options enabled after seeing your post. Makes a big difference (IMHO). Why aren't they enabled by default? Is there a down-side?

Actually, my comments about using "mythfrontend --verbose playback" and max-user-frequency had to do with your 720p problem.

In the verbose output you posted, it is unclear what A/V sync method is ...
